Graves’ disease is the most common cause of hyperthyroidism. It has multiple forms of presentation, including some rare ones, like gynecomastia. We present the case of a 45-year-old male, without relevante previous history, with complaints of unilateral painful gynecomastia, associated with history of palpitations and weight loss, with a mammography only with benign findings and with laboratory tests compatible with hyperthyroidism with positive TRAbs. As the treatment of the thyroid disfunction began, we watched a reversal of the gynecomastia, without a ressurgence at follow-up. This case is unfrequent and should be rapidly recognized.

A doença de Graves é a causa mais comum de hipertiroidismo, tendo apresentações clínicas múltiplas, incluindo algumas mais raras, como a ginecomastia. Apresenta-se um caso de um homem de 45 anos, sem antecedentes a destacar, com história de ginecomastia unilateral dolorosa, associada a palpitações e perda ponderal, com mamografia a excluir alterações sugestivas de malignidade e com estudo analítico compatível com hipertiroidismo, ao qual se adicionava positividade de TRAbs. Com o tratamento da disfunção tiroideia com anti-tiroideu de síntese, verificou-se a regressão de ginecomastia, que se manteve no seguimento subsequente. Destaca-se este caso como uma situação pouco frequente que deve ser rapidamente reconhecida.
