Background: In the field of immunology, the inhibition of immune checkpoints, such as CTLA-4, PD-1 and PD-L1, may cause immune-related adverse events, including endocrinopathies like hypophysitis and thyroid disfunction. Clinical Case: Male, 59 years, with lung cancer treated with nivolumab (anti-PD-1) who is referred to an endocrinologist for having an altered thyroid function detected on routine tests. The patient presents fatigue, muscle weakness, loss of appetite, constipation and depressive symptoms. The laboratorial evaluation revealed autoimmune hypothyroidism and secondary adrenal insufficiency. The magnetic resonance imaging showed an increase in pituitary gland dimensions, without associated lesions. The patient was diagnosed with primary hypothyroidism and adrenal insufficiency secondary to hypophysitis and therapy was instituted, with clinical and laboratory improvement. Discussion: The case report shows the importance of recognizing this clinical entity, whose early diagnosis and timely therapy may be determinant in the prognosis and quality of life of the cancer patient.

Introdução: Na área da Imunoterapia, a inibição de checkpoints imunológicos, como CTLA-4, PD-1 ou PD-L1, pode provocar efeitos adversos, onde se incluem as endocrinopatias, em particular a disfunção tiroideia e a hipofisite. Caso Clínico: Homem, 59 anos de idade, com diagnóstico de neoplasia do pulmão, sob nivolumab (anti-PD-1), referenciado à consulta de endocrinologia por alterações da função tiroideia. O doente apresentava um quadro clínico de fadiga, diminuição da força muscular, anorexia, obstipação e humor depressivo. A avaliação laboratorial revelou hipotiroidismo autoimune e insuficiência suprarrenal secundária e a ressonância magnética aumento das dimensões da hipófise, sem lesões associadas. Diagnosticado hipotiroidismo primário e insuficiência suprarrenal secundária a hipofisite, foi instituída a terapêutica com franca melhoria clínica e laboratorial. Discussão: O caso descrito revela a importância do reconhecimento desta entidade clínica, cujo diagnóstico precoce e terapêutica atempada podem ser determinantes no prognóstico e qualidade de vida do doente oncológico.
