Toxic Adenomas treated with 131Iodine - 2 years retrospective analysis

Abstract

Radioiodine treatment (RAIT) can be used for hyperthyroidism treatment, particularly for toxic adenoma. The aim of this study was to evaluate the outcome of radioiodine treatment (RAIT) for autonomous toxic nodule (ATN) and to determine the influence of age, gender, nodule size and iodine activity in the outcomes. It was a retrospective study that included all patients with ATN treated with radioiodine, in a tertiary centre, for two years. We studied 58 patients (mean age 59.6±14.0 years), in a total of 61 treatments. The global cure rate with RAIT was 87.2%. With the most used activity, 10 mCi, cure rate was 92.6% and hypothyroidism rate was 22.2%. Treatment success with 10 mCi was higher than with 5 mCi (72.7%), although not significant. None of the clinical or demographic variables have shown influence in outcome. The cure rate with 10 mCi is similar to those described in the literature for higher activities. Regards hypothyroidism, frequencies are comparable to those in low activities studies.

O iodo radiativo (RAI) pode ser utilizado para o tratamento de hipertiroidismo, em particular nos adenomas tóxicos (AT). O objetivo deste estudo foi avaliar a eficácia do RAI para tratamento de AT e determinar a influência da idade, género, dimensões do nódulo e atividade de iodo administrada, no resultado terapêutico. Foi um estudo retrospetivo, que incluiu os doentes com AT submetidos a tratamento com RAI, num centro terciário, durante 2 anos. Foram avaliados 58 doentes (idade média 59,6±14,0 anos), num total de 61 tratamentos. A taxa global de cura com RAI foi de 87,2%. Com a atividade mais utilizada, 10 mCi, obteve-se cura em 92,6% dos doentes e a taxa de hipotiroidismo foi de 22,2%. O sucesso terapêutico com 10 mCi de RAI foi maior do que com 5 mCi (72,7%), apesar de esta diferença não ser significativa. Nenhuma das variáveis clínicas ou demográficas estudadas influenciou o resultado terapêutico. A taxa de cura com 10 mCi assemelha-se ao descrito na literatura para atividades mais elevadas. Quanto ao hipotiroidismo, a frequência enquadra-se nos resultados de estudos com baixas atividades.
