Statins and Diabetes Mellitus - what is the link?

Abstract

Objectives: To review the most recent literature on the link between statins and the risk of developing Diabetes Mellitus, the underlying mechanisms, the factors that influence this link, and the implications for clinical practice. Review methods: Search on Pubmed and Cochrane Library databases, using the terms “Statins”, “Treatment”, “Diabetes”, “Risk” and “Mechanisms”. Classical and systematic reviews and original articles were included, from the last 5 years, which assess the risk of developing Diabetes Mellitus in patients on statin therapy. Results: The most recent evidence suggests an association between the use of statins and an increase by 9-13% in the risk of developing Diabetes. The underlying pathophysiological mechanism is not yet fully understood; however, the increased risk of Diabetes resulting from statin treatment appears to be related to the decline in pancreatic insulin secretion and the rise in insulin resistance. Many studies consider this risk as a class effect, yet several factors that influence this link are described, such as statin type, dose, time of treatment, and pre-existing risk factors for developing Diabetes. Conclusions: Despite these results, the benefit of statins in cardiovascular prevention is unquestionable and outweighs the risk of increased incidence of Diabetes. However, there are some recommendations to consider when prescribing a statin, accounting for the cardiovascular risk and the risk of developing Diabetes for each patient.

Objetivos: Rever a literatura mais recente sobre a relação entre o uso de estatinas e o risco de desenvolvimento de Diabetes Mellitus, os mecanismos subjacentes, os fatores que influenciam esta relação, assim como as implicações na prática clínica. Métodos: Pesquisa nas bases de dados Pubmed e Cochrane Library, utilizando os termos “Statins”, “Treatment”, “Diabetes”, “Risk” e “Mechanisms”. Foram incluídos revisões clássicas e sistemáticas e artigos originais dos últimos 5 anos, que avaliam o risco de desenvolvimento de Diabetes Mellitus em doentes sob terapêutica com estatinas. Resultados: A evidência mais recente sugere a associação entre o uso de estatinas e o aumento em 9-13% do risco de desenvolvimento de Diabetes. O mecanismo fisiopatológico subjacente ainda não se encontra totalmente esclarecido; contudo, o aumento do risco de Diabetes secundário ao tratamento com estatinas parece estar relacionado com o declínio da secreção pancreática de insulina e o aumento da resistência à insulina. Muitos estudos consideram este risco como um efeito de classe, no entanto são descritos vários fatores que influenciam esta relação, como o tipo de estatina, a dose utilizada, o tempo de tratamento, assim como a pré-existência de fatores de risco para o desenvolvimento de Diabetes. Conclusões: Apesar destes resultados, o benefício das estatinas na prevenção cardiovascular é inquestionável e sobrepõe-se ao risco de aumento da incidência de Diabetes. Contudo, existem algumas recomendações a ter em conta aquando a prescrição de uma estatina, atendendo ao risco cardiovascular e ao risco de desenvolvimento de Diabetes de cada doente.
